With the aim of making the claim notification procedure easier, we have prepared some basic rules which, if followed, will save you potential trouble and speed up the entire claim settlement process. Moreover, this website allows you to search for appropriate claim notification forms which can be downloaded, filled in and sent to the insurance company branch address.
Motor Casco Claim Notification Procedure
- In case defined by the law, each accident should be reported to the Police immediately (show) (hide). Please specify the name and address of police unit or enclose the copy of police certificate of claim notification.
- If the participants of accident didn’t agree on fault,
- If injury or death of person was involved,
- Leak of dangerous substances occurred,
- There was a damage of a road or other public facility,
- If any of the drivers is apparently under the influence of alcohol or other addictive substance,
- if the participant of car accident didn´t stop the motor vehicle or left the place of car accident
- If any of the participants of a car accident refused to share their identity to other participant or didn´t provide their motor casco insurance details,
- In case of the theft of vehicle or theft of its parts, the burglary or the robbery,
- In case of vandalism, the damage of the motor vehicle by other person.
- In case of not roadworthy motor vehicle, please call the assistance company on telephone number +421 2 444 53097 which provides you with the accommodation et al.
- In case of loss occurrence, please fill in the Traffic Accident Protocol together with the other participant of car accident (form “Traffic Accident Protocol”).
- Please note the identification data of participants of car accident and the licence plate numbers or potential witnesses.
- Please notify the insurance company of the loss on the relevant clam notification form immediately. Do not forget to include your telephone number or e-mail address to speed up claim settlement.
- Prior to the first vehicle inspection, the condition of the damaged vehicle caused by the loss occurrence must not be altered with the exception or urgent safety, sanitary or other serious reasons.
- The loss adjuster will approach you to arrange the motor vehicle inspection. After the Motor Vehicle Damage Report is drawn up, the insured person shall submit a copy of this report to the car-repair shop which will repair the damaged vehicle.
In case further damage is subsequently found in a car-repair shop, the insured person is obliged to arrange for another vehicle inspection with a loss adjuster.

Motor Casco Claim Notification Procedure - occurred abroad
- Always call the Police.
- In case of not roadworthy motor vehicle, please call the assistance company on telephone number +421 2 444 53097 which provides you with the tow away motor vehicle, accommodation et al.
- If the Police decline to investigate the car accident, please fill in the Traffic Accident Protocol together with the other participant of car accident (form “Traffic Accident Protocol”).
- Please sign the Traffic Accident Protocol only if you understand it and agree with it. Otherwise, provide your standpoint on the traffic accident in Slovak language.
- Please note the names, licence plate numbers and other identification data of participants of car accident and the name of insurance company with which the party at fault concludes TPL Motor Insurance.
- Prior to the time any repairs (incl. provisional) or transport of not roadworthy motor vehicle to the Slovak republic, please ask the insurance company for standpoint on further steps by phone or by e-mail.
- In case of doubts, please contact the insurance company on telephone number +421 55 6826120, + 421 2 57102869.
- Please send the filled in and signed claim notification form to the insurance company branch address.
Travel Insurance Claim Notification Procedure
- Please notify the insurance company of the loss on the relevant clam notification form immediately. You can also ask for help the insurance broker or the responsible person in your company.
- In case of loss (sickness, accident) you can ask for help the assistance company on telephone number: +421 911 803 500.
- Please fill in the claim notification form carefully and do not forget to include your telephone number, e-mail address and bank account number to speed up claim settlement.
- Please also enclose all necessary documents relating to trip and loss (sickness, accident). The list of necessary documents includes the insurance policy as well.
- In case of property or additional liability insurance please take photos if it is possible. It will help you to prove the extent of loss.
- In case of liability insurance is always needed to provide the written request of claimant (injured/damaged person) for indemnity.
Motor Casco Claim Notification Procedure -THEFT OF MOTOR VEHICLE
- Always call the Police.
- In case of theft of motor vehicle occurred abroad, please call the assistance company on telephone number +421 2 444 53097 which provides accommodation, transport of persons to Slovakia et al.
- Please notify the insurance company of the loss immediately and provide us with your contact details (telephone number or e-mail address).

Property Insurance Claim Notification Procedure
- Please notify the insurance company of the loss on the relevant clam notification form immediately. In case of doubts, please ask the loss adjuster which form should be filled in.
- Please enclose all necessary documents to the claim notification in respect of loss.
- Please take photos after loss immediately if it is possible and send it to insurance company in digital form. It will help you to prove the loss occurrence and extent of loss.
- After receiving of claim notification form, our loss adjuster will approach you about the place and date of claim inspection. Please do not forget to include your telephone number or e-mail address.

Liability Insurance Claim Notification Procedure
- Please notify the insurance company of the loss on the relevant clam notification form immediately.
- Please enclose all necessary documents in respect of loss to the claim notification, especially the written request of claimant against insured person to indemnity with specification of amount (extent) and the reason of loss.
- In case that it is necessary to remove results of loss because of urgent safety, sanitary or other serious reasons (food that can go rotten, glass etc.), please take photos after loss immediately if it is possible and send it to insurance company in digital form. It will help you to prove the loss occurrence and extent of loss.
- In other cases, after receiving claim notification form, our loss adjuster will approach you about the place and date of claim inspection. Please do not forget to include your telephone number or e-mail address.
Personal Accident Insurance Claim Notification Procedure
- Please notify the insurance company of the loss immediately after the completion of treatment.
- The personal accident claim notification can be downloaded from this website or you can ask the customer centre staff to send you the form.
- Please fill in the first page of the form carefully and do not forget to include the signature of the insured person or of the authorized person. Make sure you list all personal accident policies you conclude with our insurance company. If you wish to have indemnity transferred to your bank account, please state your bank account number.
- Please ask your attending physician to fill in the other side of the form after the completion of treatment.
- Please send the filled in and signed claim notification form to the insurance company branch address.
- To speed up claim settlement, please also enclose copies of medical reports relating to your accident (including up-to-date medical notifications issued after the completion of treatment).
